How we work
The 3-Phase Resilience Roadmap
-

Find the Root Cause
A thorough one-on-one evaluation to uncover why the pain keeps coming back, not just chase symptoms. We calm the pain fast so you can move without fear.
-

Rebuild Strong Mechanics
We target the real problem with custom corrective exercise, strengthening, and mobility work so you move better and build lasting resilience.
-

Return to Full Training
Everything you rebuilt gets plugged straight back into your sport or workouts. You leave with confidence and a lifetime toolkit so it does not come back.

Meet your PT
Justin Bonzato
Physical Therapist
Former Marine Special Operator (MARSOC)
I joined the Marine Corps at 18 and went into special operations. Multiple deployments left me with torn shoulders, shredded knees, and a back and hips that screamed after every ruck. Doctors said “Rest and you are going to need a hip replacement at 40.” I said “There’s gotta be another way!”.
Now at 38 I’m still running trails, rolling jiu-jitsu, and lifting heavy — pain-free. That same system is exactly what Peak Performance PT brings to fitness athletes in eastern Iowa.
CONDITIONS AND PEOPLE WE TREAT
BY BODY REGION
-
Rotator cuff pain and tears, shoulder impingement, frozen shoulder and stiffness, labral irritation, biceps tendon pain, overhead-athlete shoulder pain (snatch, jerk, press, kipping), AC joint pain, and general shoulder weakness or instability.
-
Neck pain and stiffness, cervical radiculopathy (nerve pain into the arm), mid-back and rib pain, cervicogenic headaches, and desk-posture-related neck and shoulder tension.
-
Low back pain (acute and chronic), lumbar radiculopathy and sciatica, SI joint pain, hip impingement and labral irritation, glute and deep hip pain, hip flexor pain, and stiffness or weakness limiting your squat depth.
-
Patellofemoral (runner's) knee pain, patellar tendinopathy (jumper's knee), IT band syndrome, knee osteoarthritis, meniscus irritation, and post-surgical knee rehab.
-
Ankle sprains and chronic ankle instability, Achilles tendinopathy, plantar heel pain and plantar fasciitis, shin splints (medial tibial stress syndrome), calf strains, and limited ankle mobility affecting your squat.
-
Tennis and golfer's elbow (lateral and medial elbow tendinopathy), grip pain, wrist pain from front rack and overhead positions, and general wrist and hand pain after injury.
-
Tendinopathy is its own animal and we treat it with a specific loading approach rather than rest. Achilles, patellar, gluteal, hamstring, rotator cuff, and elbow tendons all respond to the right dose of load.

FAQs
-
No referral needed. You can schedule directly and be seen for a full 60-minute evaluation with a Doctor of Physical Therapy. Iowa is a direct access state, so you can start care without a referral.
-
Cash-based means your care is not dictated by insurance. Visits are 60 minutes, one-on-one, focused entirely on your goals, not a billing code. We provide superbills upon request so you can seek out-of-network reimbursement from your insurer. HSA and FSA accepted.
-
Not at all. The majority of our patients are active adults, CrossFit members, runners, gym regulars, weekend warriors, who love training and do not want to stop. You do not need to compete professionally to work with us
-
Every case is different, but we give you a clear picture on your first visit. Most patients see meaningful improvement within the first few visits. Our goal is to get you better and keep you there, not to keep you coming back indefinitely.
